If you wish to Opt-out of the medical plan come into the UPEI Student Union on any Wednesday, Thursday, or Friday before October 2, 1998

If you are covered by another Medical Insurance Company, above and beyond Provincial Medicare, you are eligible to opt-out of the Student Health Plan from UPEI and receive a rebate.

HOW?

|. Bring proof of your other insurance plan co che Seudent Union Office (usually a card of your other insurance or a photocopy of the card, no handwritten proof accepted)

2. Proof of full-time student starus 3. Fill out a "Waiver Form’

4. Will rake approximately 3 weeks to receive rebate trom the Accounting Office
