STUDENT MEDICAL PLAN

Upon registration all full time students become members of the Student Medical Plan and are charged the annual premium. If you are covered by a medical plan other than Provincial Medicare, you may withdraw from the Student Medical Plan.

1. After registration, rebates may be arranged at the Student Union Office - 1st floor of the Student Union Building. You need:

proof of full-time registration from UPEI.

- and - proof of another medical plan (policy name & number)

2. — If you are receiving a student loan do not wait for the loan to be processed before applying for a medical plan rebate, contact the Student Union Office for details.

3. Students interested in purchasing family medical coverage can get information at the Student Union Office. Purchase deadline is October 1, 1993.

STUDENT MEDICAL PLAN REBATES ARE AVAILABLE EVERY WEDNESDAY, THURSDAY & FRIDAY AT THE STUDENT UNION OFFICE UNTIL OCTOBER 1, 1993.

Student Medical Plan cards will be available mid-November at the Student Union Office. ;
