Please print and complete the form as completely as possible. Where temperatures are requested, utilize a thermometer to measure the temperature and, if possible, place the thermometer in a place that will give a representative temperature. The wind can be estimated using the Beaufort Scale provided above. The information will be mapped using map coordinates taken from 1:50.000 topographic maps. The participants will be supplied a photocopy of the appropriate map on which they can mark the sample site.

The UTM or Universal Transverse Mercator coordinates will then be provided to you or it will be filled in by the coordinator. When recording habitat information, specify the habitat type (pond type, woodland type, grass, etc.) and give as much specific information as possible e.g. salamander found in rotten log in maple stand of 6" average diameter.
