SURGICAL PLAN

Under your subscription to the Medical Plan, when you or any member of 3our family re‘ quires surgical treatment, in the hospital, in the doctofs office, at home, or in an emergency anywhere, Blue Cross will provide benefits on your bill to your family doctor, according to

The number of surgical operations is not lim ited, and there are very few exclusions. Example: Tuperculosls Conditions .

The Plan, with few exceptions, covers all surgi- cal operations necessary for treatment of prac- tically any disease, injury, fracture or dis- location. . '

Surgical operations for hernia, hemorrhoids, tonsils or adenoids are covered, but the sub- scriber must be enrolled in the Doctor Plan for at least six months to takeﬁadvantage of these benefits.

Childbirth services are provided for a ‘wife after nine months of subscription at the fam-

MEDICAL PLAN

Under your supscrlption to the Medical Plan, you are entitled to maximum daily benefit available only to hospitalized bed patients under treatment and to be effective on the fourth day of hospitalization continuing through to the 31st. day. This period of actual- ly 28 days benefit is available for any member of hospital admissions, providing no two ad- missions are required for related illnesses within a three month period.
